Medicare Routes Explained

Understanding Your Medicare Choices: Two Different Routes

To choose a coverage route, you must first enroll in Medicare Parts A and B. Your Part B premium is determined by your income. Because original Medicare has coverage gaps, most individuals choose one of two routes to protect against high out-of-pocket costs.

Route 1: Original Medicare + Medigap + Part D

This route provides predictable healthcare costs with minimal financial risk, though it requires higher monthly premiums.

  • The Structure: You keep Original Medicare (Part A for hospitals, Part B for doctors) and purchase a private Medicare Supplement (Medigap) plan alongside a standalone Part D prescription drug plan.
  • Estimated Cash Flow: Medigap plans typically run $150 to $200 per month, and drug plans average $35 per month. This creates a predictable total premium cost of roughly $230 per month ($2,760 per year).
  • Out-of-Pocket Risk: Hospital stays are generally covered at 100%. Your doctor visit exposure is limited to the annual Part B deductible (approximately $283), after which coverage is 100%.
  • The Trade-off: There is typically no coverage for dental, vision, hearing, over-the-counter (OTC) items, or gym memberships.

Route 2: Medicare Advantage (Part C)

This route acts as an "all-in-one" alternative that lowers your monthly fixed premiums but requires copays when you access medical care.

  • The Structure: Medicare outsources your coverage to an approved private insurance company. The insurer bundles Part A, Part B, and usually Part D into a single Part C plan.
  • Estimated Cash Flow: About 60% of these plans have a $0 monthly premium, while the remaining 40% average around $15 per month. You save the ~$230 monthly premium required by Route 1.
  • Out-of-Pocket Risk: You pay copays or coinsurance as you go. Total annual spending is legally capped by the plan's Maximum Out-of-Pocket (MOOP) limit. If you experience a high-utilization medical year, Route 2 can occasionally cost more than Route 1.
  • Provider Networks: Coverage relies on managed networks. PPO networks allow you to see out-of-network doctors at a higher cost, while HMO networks generally limit coverage to in-network providers.
  • Added Perks: Most plans bundle extra benefits like dental, vision, hearing, OTC allowances, and gym memberships and perhaps Part B premium credit.

What to Do If Your Plan is Discontinuing

If your current Medicare plan is exiting the market, you are granted a Special Enrollment Period to select a new route, much like when you first turned 65. This window presents a strategic opportunity to evaluate your coverage based on your current health status:

  • If your health needs have increased: If you face rising copays on a Medicare Advantage plan, a plan exit gives you a unique opportunity to switch to Route 1 (Medigap) without medical underwriting. This allows you to trade unpredictable copays for a steady monthly premium.
  • If you remain healthy and low-utilization: Staying on Route 2 (Medicare Advantage) may allow you to continue saving thousands in premium costs annually while preserving extra benefits like dental and vision.

The choice is entirely yours, and evaluating your options ensures your coverage aligns perfectly with both your health and your budget. Use a Licensed and Medicare Certified Agent to assist; volunteers have no liability for advice.
